What We Expect From Patients

We expect our patients to be polite and courteous to our staff, and endeavour to contact the surgery if they are unable to keep their appointments.

It is not our intention to remove patients from the practice list, however on occasion we may have to due to inappropriate behaviour. We do not accept violent or abusive behaviour towards any member of staff. If we intend to remove you from our list we will write and inform you and NHS Hillingdon.

Changing Personal Details

Please inform us of any changes to your personal details such as name, address, telephone number (s), and email address to enable our records to be as up to date as possible.

Patient Survey

A patient survey will be performed at least annually. The results of the survey will help us to continually improve our services.

Patient Participation Group

As the practice is new we would like to offer our patients the opportunity to join a patient participation group. We envisage meeting every eight to ten weeks to discuss service provision and development. If you are interested in becoming involved in the group please leave your details at the reception desk or let us know by telephone or email.

Confidentiality

The Practice manages the confidentiality of your medical records in accordance with the Data Protection Act 1998. Please note that medical records are subject to inspection by NHS Hillingdon or its equivalent, for the purpose of financial audit, record validation and research. Should you wish your records to be excluded from such inspection or use, please write to the Practice Manager. Any proven breach of confidentiality will be treated extremely seriously.

Data Protection

The Data Protection Act 1984 and Access to Heath Records Act 1990 give patients the right to see medical records made after November 1991. Any such request must be by appointment and may be subject to an administration charge.

Complaints

All employed staff, including community or outside attached staff, are obliged to comply with practice policies. If you feel you have reason to complain, please ask reception for a copy of the Complaints Procedure or speak with the Practice Manager, Alison Somers.
